The application must be made in writing in Germany, Iceland, Mexico (registered letter), the Netherlands. In the case of Belgium, Spain and Denmark a form has to be filled in. In Luxembourg, the application may be made on a sheet of paper or a form. In the case of Hungary, it must be made in person or by a person delegated for that purpose, or by registered mail. In Malta, the registration requires a sworn declaration, while in the case of Poland the application may be made in writing, by telephone, by telex or by fax.
