Article 11
 
1. A permanent register of voters shall comprise all persons that permanently reside on the territory of a commune and have the right to vote.
2. A person may be entered on one register of voters only.
3. The electoral register shall be used to prepare the list of eligible voters for elections of the President of the Republic, elections to the Seym and to the Senate, elections to the European Parliament and elections to commune councils, district councils and voivodeship councils, elections of head of  commune, mayor and president of town as well as lists of those having the right to participate in national and local referendums.
4. The register of voters shall be divided into Part A and Part B.
5. Part A of the register of voters shall be comprised of the citizens of Poland. In this Part of the register of voters there shall be inserted: the surname and forenames, father’s name, date of birth, the identity document (PESEL) number, and the address of the elector.
6. The voters, who are citizens of Poland and are registered  on the territory of a commune as permanent residents, shall be entered ‘ex officio” in the register of voters.
7. Part B of the register of voters shall comprise citizens of the European Union, non-nationals of Poland, who permanently reside on the territory of a commune and entitled – on the basis and in the scope of provisions of other acts of law to exercise of the right to vote in the Republic of Poland. This Part of the register of voters shall comprise the surname and forenames, father’s name, date of birth, citizenship of Member-State of the European Union, number of passport or other identity document, and the address of the voter.
8. The voter, referred to in paragraph 7, not being entered in the register of voters jet, shall be inserted upon his/her motion into the register of voters no later, than 30 days after the date of issue of the order on elections, in which, on the basis and in the scope of provisions of other acts of law he/she intends to participate.
9. To the motion, referred to in paragraph 8, there shall be amended:
1) a xerox copy of a valid identity document of the mover, and
2) a written declaration, in which the mover have to state:
a) his/her citizenship and the address of permanent residence on the territory of the Republic of Poland;
b) the name of a electoral constituency or locality in a home Member State where he/she is inserted in the register of voters;
c) a declaration, stating that he/she intends to exercise his/her right to vote on the territory of the Republic of Poland only;
d) a declaration, that in a home Member State  he/she has not been deprived of his/her voting rights.
10. The commune shall currently maintain the register of voters as a task prescribed to it.
11. The register of voters shall be open to inspection, on written request, in the office of the relevant commune.
